The passing of a loved one can be one of the most difficult things we will ever face, and making the many decisions which come at a time of loss can be overwhelming.

Call us and inform us of the passing. We will need to know where your loved one is located so we can respectfully take them into our care. Our 24 hour phone number is 1-800-931-9880. Rest assured, when you call we will walk you through the entire cremation process.

Many entities require that an original death certificate be submitted for notification or to close accounts. 1 copy of the Death Certificate is included with the Cremation package. For additional copies, make your selection here.

The option for shipping is included with the Cremation package. Use this option only if the cremated remains need to be shipped to more than one destination.

Dallas County Medical Examiner’s Office Release Form
This form is only necessary if your loved one is currently at the Dallas County ME’s office.
Tarrant County Medical Examiner’s Office Release Form
This form is only necessary if your loved one is currently at the Tarrant County ME’s office.
US Flag for Burial Purposes
This form is only necessary if your loved one was a member of the military and might be eligible for a US burial flag. Read instructions on page two of the form for submission options.
Military Discharge form DD214
This form is only necessary if your loved one was a member of the military and you need the records to access military benefits. Form will be filed on-line.
